The goal of that accompaniment lesson is to learn how to stay on one hole of the harmonica and be able to hit a note of the chord you're playing over.
For example, on hole 1, the I Chord is G B D. On your C harmonica, 1 draw is D, 1' is Db, and 1+ is C. So, 1 draw D matches the I Chord. For the IV Chord, 1+ matches. For the V Cord, the 1 draw and 1+ matches. This is what you fill in for your first column. Take a look at that accompaniment lesson with this in mind.
