In the opening of the movie Once Upon a Time in the West, within the first minute, it sounds to me almost like Franco rattle the reeds.

Does anyone know what he does, and/or how he does it?

Half-slide

he's using a chromatic and has the slide halfway in so youre hearing two reeds that are tuned a semitone apart, one reed from the key of C reed bank, and one form the key of C# reed bank.
