David,

The E (5+) on my C harm sounds more closed, more flat and less volume than the other notes in the same octave.

It sounds difficult.

What can be the cause?

Hello Mertens. The 5+ is not

Hello Mertens. The 5+ is not commonly prone to player-caused problems, so it's most likely the harp. There could be something in the reed slot (food/hair) or less commonly the reed could be misaligned or has failed (fracture at the base of the reed). I recommend you take the cover plate off and look to see if anything is impeding the reed. If there is nothing then take a look at Harp Tech Study 2 - Maintenance & Tools http://www.bluesharmonica.com/harp_tech_studies_2 to learn how to adjust your harmonica.
